Notice: Out of School Hours Care (OSHC) Expression of Interest

Yarra Glen Primary School is currently seeking expressions of interest from licensed OSHC providers to deliver a service from the school site, commencing January 2025. This process is being undertaken as a result of the expiry of our Out of School Hours Care (OSHC) license agreement with the current provider.

EOI applications expire 30th August, 2024. Site tour scheduled for 23rd August at 11.30am; meet at the school office.

Enrolment Forms

If you are looking to enrol your child at Yarra Glen Primary, please download the enrolment forms (below) or contact our office on 9730 1254 for an enrolment pack to be sent to you, and then return the completed forms to confirm your child’s enrolment. We require a copy of your child’s birth certificate and immunisation history statement (whether they have been immunised or not) to complete the enrolment. A copy of your child’s immunisation statement can be obtained from your Medicare online account (through the myGov or Express Plus Medicare mobile app – please refer to the pamphlet below). We look forward to seeing you and your child in the near future!

Medications & Plans

If your child has anaphylaxis, asthma or allergies, please fill in the appropriate ASCIA form below and have it signed by your GP. These forms are to be completed annually or if your child’s medical condition changes and must have a recent photo of your child attached. Medication must also be supplied in a labelled zip lock bag with your child’s name and class.

If your child is required to have medication during the school year (for example, antibiotics), please fill in and sign the medication authority form (below) and send it with the medication to the front office.
