If you are looking to enrol your child at Yarra Glen Primary, please contact our office on 9730 1254 or email yarra.glen.ps@education.vic.gov.au for more information. New prep enrolments for the 2026 calendar year complete an online application on the new Insight portal. Students transferring interstate or from a non-government school will need to complete the student enrolment form below. We look forward to seeing you and your child in the near future!
Medications & Plans
If your child has anaphylaxis, asthma or allergies, please fill in the appropriate ASCIA form below and have it signed by your GP. These forms are to be completed annually or if your child’s medical condition changes and must have a recent photo of your child attached. Medication must also be supplied in a labelled zip lock bag with your child’s name and class.

If your child is required to have medication during the school year (for example, antibiotics), please fill in and sign the medication authority form (below) and send it with the medication to the front office.
